Application for Consulting

Thank you for your interest in working with me. I know that the Lord can help us as we work together to address the things in your life that are holding you back from the fullness of what He has for you.

Before we can begin you will need to fill out an application and to agree with the terms and conditions of working together. As mentioned previously, I am not a licensed counselor and will not be providing any form of psychiatric or therapeutic counseling. I will be working with you as a spiritual “consultant” where we will work together with the Holy Spirit to deal with specific issues that you have identified.

The first step in this process is to fill out and return a release form. You can download the file by “right-clicking” here and then save it to your computer. You will then need to print it out, sign and date it and then return it to me either through sending it to my contact form or faxing it to 757-455-8638.
